Cook the Sausage:
In a large skillet over medium heat, cook the ground sausage until fully browned, breaking it up with a spoon. Drain excess grease if necessary.

Reduce heat to low. Add softened cream cheese to the skillet and stir until melted and combined with the sausage.
Add Remaining Ingredients:
Stir in shredded cheddar cheese, sour cream, garlic powder, onion powder, black pepper, and red pepper flakes (if using). For a creamier dip, add cream or milk gradually until desired consistency is reached.
Simmer:
Cook on low heat, stirring frequently, until all ingredients are fully combined and heated through, about 3–5 minutes.
Serve:
Transfer to a serving dish and garnish with chopped green onions if desired. Serve warm with tortilla chips, crackers, or sliced vegetables.
